Esta revisão descrever as alterações fisiológicas que ocorrem nos sistemas respiratório e cardiovascular como parte do processo de envelhecimento e avalia a aptidão cardiorrespiratória (ACR), destacando o VO2pico como possível biomarcador de fragilidade em pessoas idosas (PI). A evidência disponível indica uma relação inversa entre o VO2pico e a fragilidade, sugerindo que este parâmetro, ao refletir o estado funcional dos principais sistemas fisiológicos, poderia desempenhar um papel crucial na identificação precoce desta condição. Isto é particularmente relevante, uma vez que a detecção atempada permitiria o desenho de estratégias de intervenção preventiva personalizadas, focadas na melhoria da funcionalidade física e na redução de riscos como quedas, hospitalizações ou perda de autonomia nos PI. No entanto, embora os resultados atuais sejam encorajadores, ainda são necessários estudos longitudinais e ensaios clínicos bem desenhados para estabelecer com maior certeza a utilidade clínica do VO2pico como marcador prognóstico e guia para intervenções terapêuticas.
